A) Yeah, it's certainly not a file-name length issue---I don't personally put all that much info in the file name, as shown in the example.
B) I do have autoload turned off in my OS environment, I assumed this would solve the issue as well. I tried again with the L-shift trick (which I had forgotten about, having not used it since 2002 or so), and encountered the same error.

BTW, hold down the Left <Shift> key when you load the disc. Don't let up on the key until CDex has begun searching the CDDB for a few seconds. Holding down the left SHIFT key prevents certain PROTS from loading.

I've been able to rip most copy protected discs. There are only a very few in the years I've been ripping my discs that I've been unable to rip due to copy protection. There have been various ways around that.
Is the complete file name "The Magnetic Fields - 01 I Die.mp3"?
I use file names such as Artist-Year-Album-Track #-Song Title.

Choosing "Reencode compressed audio files" with identical folders for infiles and outfiles causes CDex to give and error message stating this, and crash.

I've encountered this before. I believe it occurred when I had another program using the same disc drive. Perhaps one or both programs were set to lock the drive for use by only one program. Never the less, I got the native SCSI message. Closing both programs and restarting CDex solved the problem. If I'm remembering incorrectly then rebooting and restarting CDex worked. I think the former...

I'll give you a clue.
This will give you an idea of the problems.
