There should be a way to flag individual headings for
not being displayed in the TOC. The reason is that even
though a subheading might improve readability of a part
of the text, it may do nothing good for the overview of
the entire document.

Consider, for example,
http://en.wikipedia.org/wiki/2004_Democratic_National_Convention
(7th of August revision). Here, the three "Quotes"
subheadings and the subheadings under each date of the
"Speakers" section clutter the TOC and don't add much
meaningful information.

I believe the best markup for this would be a keyword
at the end of the line of the heading, e.g.:

===Official sites === HIDE_FROM_TOC
...
===Fan sites=== HIDE_FROM_TOC
...
