in instruction #13 say to enter multiple host names in the Server Name
field, but

1) it does not say whether to comma or space-separate them
2) even after comma-separating them, only www.domain.com works,
domain.com does not route to the same place.

In the VirtualHost section, set ServerName to one (domain.com) and set ServerAlias to all the others (www.domain.com).
