Sounds exactly like the issue I was having.  Is the other environment using AD as well?  Is PAM_ldap set up the same (or are you using sssd)?  When I went looking before I gave up, it wouldn't work if I selected LDAP from the dropdown yet the LDAP user module was able to do lookups just fine.  I'd have to dig through the archives to find Jamie's response (pretty sure he did respond).  I don't post much so it should be a fairly easy search :-).

RedHat 6 servers, trying to use Active Directory credentials. We have it working in another environment, so I’ve been trying g to look at differences between the two servers, but I can’t seem to put my finger on it. Neither Webmin nor Usermin are functioning properly at this point.

Are you trying to use LDAP accounts by any chance?  I've yet to get that to work for me as webmin user accounts.

Subject: [webmin-l] Issues setting up PAM authentication

I am setting up a new Webmin/usermin instance and am running into an issue with getting PAM authentication working.

I have installed the Authen::PAM perl module, and have configured Webmin to use PAM authenticatyion when possible, but when I try to log in with anything other than the initial admin user, I get a 401 response code.

I don’t see anything other than the 401 in any logs & am not sure how to troubleshoot this one. Any ideas?
