This doc has pages numbered
  i - xx
  1 - 689

Nowadays it is custom to begin the arabic numerals from where the 
roman numerals left off, so that printing subranges of pages works 
properly without the user having to correct for the offset caused by the 
initial roman-numeral pages.

So, the pages should be numbered
 i - xx
 21-709
or why not simply
 1 - 709

Printing subranges of pages from Apple's Preview app does not work
properly without the user having to correct for the offset caused by the
initial roman-numeral pages.  (I didn't try Adobe Reader until now, and I
see that it displays both the page number of the page and the sequence
number for use with print ranges, so it's not a problem with Adobe
Reader.)

Sorry I don't have chapter and verse on the Roman numeral numbering scheme
I advocate.  It's just usage I started to see in the modern era of
electronic composition of documents.

The pdf documentation is generated by the htmldoc tool. The page numbering
you see is the default output from this tool. If htmldoc supports this
numering scheme, I don't mind changing the page numbering to whatever the
modern way is, but do you have any supporting evidence for this? The
ISO/ANSI C and C++ standards for example use the same approach that we do.
I'm not entirely sure what is wrong with the current approach, it makes
more sense for the body to start at page 1 than some arbitrary number
depending on the length of the table of contents. Acrobat handles the page
numbering nicely so there is no problem there either ... ie if I type in
vii or 25, it will take me to the pages numbered vii or 25 respectively.

Actually, it's worse than that.  The title page has no numbering at all.
