#10 indentation cleanup in plain dtml files

You might just reject this one out of hand, as it's almost purely cosmetic changes
to make the dtml/html a bit more readable. However, there's one instance of some
code cleanup, too. In Squishdot_previewForm.dtml, it seems you can drop about
500 bytes of code by putting the following in place of the repetitive if/call statements:

<dtml-var standard_html_header>
<dtml-in "['title','author','body','email','notify','summary','dept','subject']">
<dtml-let item=sequence-item>
<dtml-if "not REQUEST.has_key(item)">
<dtml-call "REQUEST.set(item,'')">
</dtml-if>
</dtml-let>
</dtml-in>
