Okay, I couldn't find any instance of this, so long as the version of SM was 1.0pre3. Anything before that and it flakes out exactly as described. Checking the SM website and my own README, no mention of this requirement was made, since 1.0pre3 adds the 'compose_form' hook. So I put that in my README and I'd recommend adding that to the Plugins page as well. Now, a simple 'README' update wouldn't warrant a new release. Luckily, there actually were a couple of small logic errors ;) now resolved. In case you're wondering, one prevented recovery of your email if you left the page after less than 15 seconds, the other was causing two backups to run simultaneously on emails that had been restored.
