 for more detail, it's size is 146GB, and it has only one partition:

 "HARDWARE IMPENDING FAILURE TOO MANY  BLOCK REASSIGNS" what this really mean?

Well, it means what it says. Your second SCSI hard drive, /dev/sdb, is
reporting that it is about to fail. You should replace it before it
actually does fail, and restore your data onto your new drive from your
backups. If you don't have any backups, make one now!
