#86 Upgrade additions

This is a diff (made by doing diff -N -U3 -xCVS -
r /tmp/slash/sql/mysql/upgrades /usr/local/src/slash/slas
h/sql/mysql/upgrades ) of the upgrades file to
incorporate things that are missing when upgrading a
site going from fry -> devel.

I had to put these additions at the *top* of the file,
otherwise it breaks things. (example: lower in the file is
an insert to string_param, so the create for that table
needs to be ahead of anything that uses the file).

Hope this helps. Adding this to the upgrade file has
definitely got me further along in upgrading a fry site ->
devel.
