I believe Yaron proposed a tutorial (http://yaronkoren.com/smw-tutorial-video/) in the semantic forms group list (http://groups.google.com/group/semantic-forms/browse_thread/thread/6e4b16b57523a6f6?hl=en) but I'm not sure about its status.

IMHO, its an important quiver in our SMW evangelization efforts that's sorely missing.
