SB-EXT:RUN-PROGRAM is probably the thing you are looking for:

The implementation in src/code/run-program.lisp seems to contain Windows-aware stuff. However, it does not support some additional options. But it doesn't look totally broken on Win32.

Dear SBCL/Windows developers,

I'm looking into making XCVB universally available. For that, I need
all Lisps to be able to invoke xcvb in a subprocess via run-program or
equivalent. This functionality doesn't seem available on SBCL for
Windows. Could a SBCL hacker using Windows implement it, maybe getting
inspiration from code with similar functionality in CCL?
