#901 -expand:off with DTDs on .NET

The command line -expand:off is designed to suppress expansion of attribute values defined in a DTD or schema. It always works in the case of a schema, because Saxon is then in control, but in the case of a DTD it depends on the XML parser in use.

The switch isn't currently working on the .NET product when the Microsoft XML parser is used; however it can easily be made to work, and a patch is being issued in Subversion (on the 9.1 branch) to fix this. Affects module DotNetPullProvider.
