Thanks for the help. I did have an issue with page setup however on solving that it appears that I'm still stuck on what appears to be a basic import. It can't detect the pages.

I've put all the detail below.

Also I've noticed vastly different results by tweaking the sharpness, density etc settings of my scanner. Are there any recommendations on what works best?

When importing OriginalScanned.pdf (same file I used for page setup) I get the following output:

I added the temp barcode filename line - it saves out the cropped image used to detect the barcode - you can see the images in the zip file from rapidshare.
You can see the image bar3b.tmp.png - it looks like it should be able to detect a barcode there however the code says INVALID - IGNORING BLANK PAGE.

Even if I proceed and then upload the OrionSmallScanned.pdf when I "verify form" there is a major downward offset of the place where it's looking for the marks. Also on other forms even if they were aligned perfectly there was still no detection of marks. See capture_offsetdownwards.jpg for example.

I'm sure these are very simple problems with someone who knows what's up here but I'm completely stumped.

If someone could assist that would be fantastic.

I've just installed the latest version of QueXF (1.8.1). It's a great piece
of work!

However I can't for the life of me get it to use any forms besides from the
ones that are provided as an example.

I have generated PDF's and captured bands but either on import the barcodes
are not recognised or the bands are misaligned when capturing.

As an example when I try and import the attached file (scanned at 300dpi in
b&w) it doesn't detect the 3rd page and infact generates an error:

I can import the pdf if I don't scan it (attached Original.pdf) first but
then it won't scan the filled out forms correctly - the bands are misaligned
by a large amount.

Does anyone have some pointers to the error and also why the bands would be
misaligned?
