CPdf::trailer cleanup

This is just cleanup change to remove CPdf::trailer field to make further
changes to support trailer editing easier.

CPdf::trailer is currently used only to initialize docCatalog and in fact it
is not required for it because XRef exports methods getRootNum and
getRootGen to get this reference.

Thus this field is used only in CPdf::getTrailer method and this one can
return the fresh (from XRef::getTrailerDict()) everytime this is called.

The advantage of this approach is that we don't have to keep XRef
trailer and CPdf::trailer in sync when we enable trailer editing.

Fix SEGV in delinearizatorTC fail path

We cannot deallocate writer if delinearizator creation fails, because it is
already deallocated in the ~Delinearizator -> ~PdfDocumentWriter when
unrolling NotLinearizedException from Delinearizator constructor.

Workaround for variable number of macro parameters.

ANSI C++ doesn't define variable number of parameters for macros. GCC
implements this as an extension but many other compilators don't do this
(e.g. the one from visual studio).

This simple hack workarounds this by multiple parameters coupling into one
preprocessor token which is provided to the function call along with
parentheses which looks like a function call after macro expansion. I have
added simple access macros NO_PARAM, PARAM_1, PARAM_2 to hide the actual
hack.
