[PATCH] Add support for Power5+

The attached patch adds support for the IBM Power5+ processor.  This 
processor has more events than its Power5 predecessor, thus it requires 
a separate set of events and event_mappings files.   I added a blurb in 
the doc to indicate that a 2.6.16 or greater kernel version is required.
