#53 Yamaha SFG-01/SFG-05 support: MIDI and keyboard (YM2148)

The SFG-01 and 05 are synth modules with MIDI interface
that are built in the Yamaha CX-5M e.g., but can also
be plugged into any other MSX. The FM chip is the
Yamaha YM2151, which seems to be emulated in MAME
already (again written by Jarek Burczynski). An

Apparently there is also the YM2164. I'm not sure what
that is exactly.

Support for the Yamaha SFG-05 has been partly implemented: the YM2151 is emulated (including the SFG-05 device), but still missing is the IM2 stuff in it, the MIDI and the keyboard support.

IM2 has been implemented. Still TODO: MIDI and keyboard support (YM2148), to make it more usable.

Note that this MIDI could possibly also be used for the Philips Music Module.
