Wesley, since you don't specify, I'm assuming you're using NMS FormMail.
You need to use the field names of 'realname' and 'email' in your
form. This is covered in the README file (lines 478-488, or
thereabout). The documentation files (README and EXAMPLES) are bundled
with the program code, as well as viewable online:
