#124 blank pages wrong with non standard paper size

Adding blank pages with a PDF file with non standard paper size (such as http://pdf.codev2.cc/Lessig-Codev2.pdf\), includes a page with a different (standard, I guess) paper size. Blank pages have the same paper size when the other pages have an standard paper size.

This has another undesired effect, namely that a java.lang.NullPointerException error when trying to impose it on a standard paper (at least with options -nup 2 -paper A4).

I guess these are bugs that should be fixed.
