I have compiled mod_qos 9.6 on FreeBSD 7.2 apache 2.2 and it was OK, but when ever I browse any page other than html pages, it does not show any thing and in httpd-error log, it says that:

[emerg] mod_qos(004): could not create a-mutex: File exists
,

Then I just restarted the apache and its fine,

If I use graceful mod_qos throws the above message.

Although mod_qos the shared memory after graceful, but it looks it does not doing it efficiently or leaving the files in somewhere.
[root@freebsd1 /usr/local/etc/apache22]# apachectl graceful
httpd: Could not reliably determine the server's fully qualified domain name, using 91.99.99.242 for ServerName

Below is the shared memory status:

[root@freebsd1 /usr/local/etc/apache22]# ipcs
Message Queues:
T ID KEY MODE OWNER GROUP

Shared Memory:
T ID KEY MODE OWNER GROUP

Semaphores:
T ID KEY MODE OWNER GROUP

I think there is a Bug in mod_qos for not removing the shared memory and

NOW ITS WORKING FINE .. why ?????????? Now I am doing several time graceful or restart apache .. ITS OK >......
