    like:

 ICON "IconName", ID, left, top, width, height, flags

    so, for example:

 Example ICON Example.ico
 XXX DIALOG 26, 41, 350, 242
 BEGIN
 ICON "Example", -1, 1, 221, 32, 32, WS_CHILD | WS_VISIBLE
 ...
