I have the same problem here, using TeXnicCenter. Though i played around
a bit and found that it was only affecting my normal working acccount
(it's WinXP SP2 btw). When I run Yap as another user it loads the same
doument without genrating any fonts.
From the error messages YAP produces I guess that YAP is somehow mixing

Following some intuition I started MikTeX Options from the start menu
and refreshed the filename database and update the formats from there.
Well, that solved the problem. YAP did generate the fonts as it is
supposed to do and loaded the files I had problems with.
I did a update some time before the problems occured which was about the
time YAP had to genrate a new font the first time after the update.
So I guess im someone's using YAP heavily there might be a long time
until that first occures. (Which explains people saying they haven't
installed/updated anything lately.)
