One way to get what you want is:
1. run makeindex to your file;
2. go into your working directory and find your index file: with extension ind (e.g. foo.ind)
3. edit your ind file and make the necessary changes: just write the whole alphabet letters at the right places.
4. run LaTeX on your file at least 2 times (but do not run any more makeindex because you have to go again to step 2 then.)
This is the way I do.
