I like the 2 table approach. It looks a little unwieldly, but the mantis_print_prefs_table looks like just a lookup table.

I'd also suggest adding some kind of sequence or ordering field. That way, people can change by drag-n-droping the fields. Even though it may be too much for the latest version, it'd be nice to have it in there for future use.
