It's not correct, but it may be the best we can do. GMV doesn't
support Quad9 elements, so for GMV output we refine each of them into
four Quad4 elements. This usually looks okay (just a little less
smooth than it ought to) for uniform meshes, but it can make solutions
on adaptive meshes look discontinuous when they really aren't.
