I've seen this with images with wrong icc profiles, usually an image with rgb pixels and an cmyk profile. iText doesn't process images it just uses what it is given.

I have an image in CMYK colorspace format that was converted to RGB colorspace. After conversion image cannot be rendered. Some versions of readers display "Insufficient data for an image" message while others display it but with a blacked out colorspace. It could be an issue with the image itself or iText image processor not supporting certain types. I have attached the final converted image. I am not familiar with image processing all that much but It would be great to hear ideas/pointers about how to debug this.
