choose a terminal suitable for printing, e.g. postscript or pdf.
set the terminal size to the size of the plot plus margins.
set the margins so that the plot occupies the required space.

an example:

# the plot is 10cm x 10cm, the whole document is 12cm x 12cm
set term postscript size 12cm,12cm
set lmargin at screen 1./12.
set rmargin at screen 11./12.
set bmargin at screen 1./12.
set tmargin at screen 11./12.
set xrange [0:10]
set yrange [0:10]
set output 'test.ps'
plot x
set output

when printing the postscript document the axes
should be 10cm long.
