honour removal time when deleting actors

This fixes problems with dead (petrified) actors being removed on
purge (e.g. Tamah in AR3500 in BG1). In order for this to work
properly we need to set RemovalTime when actors die, though.

Problem here was that the RentIndex var was set after all the buttons
were already set up but they were never actually redrawn.

heal right amount of HP on resting

Also change Actor::Heal() to actually interpret its parameter as
HP healed like everybody using it apparently assumed...
