You check options to include cdp in the certificate profile used to issue the certificate. You can't miss this option, it's called "crl distribution point". Regarding dn order my guess is that you are checking the option in a certificate profile, but using another certificate profile when the certificate is issued. Note that this option is for the issued certificate, not for the "user" in the database. The same option is available in ca configuration.
