According to RenderX' tech support: Unfortunately, XEP still does
not support "scale-to-fit" values for 'content-width' and 'content-
height'. There's more: 'width' and 'height' are simply ignored,
and image size is determined by 'content-height' and 'content-
width' alone. (In fact, 'width' and 'height' specify the viewport
size, not the image size; we don't support positioning images
inside viewports).

So I've changed the way width and height
attributes are handled when xep.extensions is turned
on.

    With the introduction of contentwidth and contentheight
    attributes in DocBook 4.2, I've completely rewritten this
    part of the stylesheets.

    I think the new code will "do the right thing".
