#108 fix dbfo processing instruction for entrytbl

The tgroup template in table.xsl is also used for entrytbl, but in
this case the dbfo processing instruction is a child not a sibling.
This patch should fix the problem. (Maybe there is a more generic
fix, but this is what I have used...)
