#211 ctags doesn't ignore PHP comments

I just started using ctags 5.7 and taglist 4.5 with MacVim while working in PHP, and it doesn't seem to be
skipping commented lines. As a result, lines like these:

// The public start function which runs the whole thing

// There will be a separate class for this eventually

cause "which" to show up as a function and "for" to show up as a class
in the taglist.

    Fixing this would require a full rewrite: switch from regular expressions to a lexer. Strictly speaking, since // comments are line based, one could go back to the original method of scanning a line at a time, but that obviously wouldn't work for C style comments.
