#71 Support git diff format

git diff format has the following differences from "diff -u" output:

- after every "diff" line there may be a "new file mode" line and then an "index" line
- file names are prefixed by "a/" or "b/"
- there are no dates in file headers (I already reported the requirement for tabs in file headers as a bug)

    Hi there,

    Can you attach a git diff file for me as a test case? Then I can implement the changes to the parser without having to download git myself. Try to create a diff which has new/removed files as well as modified files. Thanks.
