Centralization of the definition of the PLplot website

The name of the PLplot website (that used to be http://www.plplot.org, but is now
plplot.sf.net) is defined in configure.in, with the variable PLPLOT_WEBSITE.
It is possible to change its value through the --with-plplot-website option
of configure.

The PLPLOT_WEBSITE variable is AC_SUBSTituted in both src/plplotdoc.xml.in
and bin/api2man.pl.in.  In the master Docbook file (src/plplotdoc.xml.in),
an entit PLplot-webiste is created, which allows constructions like '<a
href=http://&PLplot-website;">PLplot</a>' in an xml source file (see
src/intro.xml). The file bin/api2man.pl has been replaced by
bin/api2man.pl.in.  The former is automatically generated by configure.

I gave up in trying to generate a Debian package for docbook2X, which is
needed for generating the info form of the PLplot documentation from the
Docbook sources.  The thing is not buildable neither installable, besides
being quite old and, apparently, not actively maintained.

There are other tools available for generating info documentation from
Docbook (like docbook-utils in Debian) but, to the best of my knowledge,
they can only process SGML sources, not XML.

Taking all this under considerations, I decided to include the necessary
scripts and modules from the docbook2X project into PLplot.  There are only
five of them.  These files are released under the Gnu GPL.  We must mention
that somewhere in the License statement of PLplot.  Also, the file
doc/docbook/README.developers has to be updated to reflect the new situation.

The files configure.ac, Makefile.am, and src/Makefile.am under doc/docbook
have been mofied accordingly.  All the tests and options regarding docbook2x
have been removed.

I am also committing a small fix in api2man.pl.in, for properly formatting
paragraphs.

Make the temporary XML file to be a standalone one, such that entity
definitions are not looked for externally.  Also, added defintions for the
entites found in ../src/api.xml (like °, <, etc.) This change may
make this script work in any situation, even in broken OS'es ;-)

This jumbo cvs commit is the result of a herculean task to update the
copyright notices of most of the source files of PLplot.

I hope that I did not introduce any serious breakage.  This seems plausible,
because I did changes only at the beginning of the files and only in
commented lines.  However, I used a semi-automatic procedure for doing that
and some bugs may have slipped into the files.  The sources still build okay
(at least the Debian packages) and I can install the libraries, and compile
and run some C examples.  That is not enough, so PLEASE TEST!

This is what I did in more detail:

1) From the CVS ChangeLog, I considered all the files that changed since the
   last release (5.2.1).  I then added the line:

       Copyright (C) 2004  Joe C. V. S. Committer 

   where the name of the developper who did the CVS commit is included.
   
2) Notice that the year in the line above should read 2004, even if the
   changes were done last year, since Copyright years refer to when the
   software is released.
   
3) For the same reason, I did not bother adding missing Copyright entries
   for previous authors of each file.  One copyright holder is enough for
   our purposes.

4) I also reformatted and reworded the License Conditions text.  This
   included changing things like "This file is free software" to "PLplot is
   free sofware" and so on.
   
5) Finally, some files were lacking License notices, especially the src/*.c.
   I added them and put in the Copyright holder line the name of the
   developer who did the last cvs commit for the file.
