You have the right idea. I've never done it this way before though. In
this case what I usually do is not to activate the PLM directly, but
instead have MH do it via BrowseCategories->Insteon->PLM Complete
Linking As Reponder.
I don't know for sure, but you might have linked the Toggle with PLM,
but not Misterhouse itself. You could try
BrowseCategories->Insteon->PLM Scan Link Tables. That might sync up MH
with the PLM.

In BaseInsteon.pm it appears that somehow the Insteon code got a blank
state in the "set". I don't know why.
