I have tested your questionnaire (Folder 1)
I do not have a problem with the form alignment, but I do seem to have
a problem with banding. On the left and right columns, queXF seems to
detect the boxes as being in-between the boxes instead of in the
boxes. This is probably due to the gaps between the boxes being
different in the left and right columns compared to the middle column.
Is it possible to adjust this?
Also, I noticed that the mark recognition was incorrect. I believe
this is due to the boxes being a different size to the standard queXML
form box sizes. You will have to adjust the following configuration

"0" filled is completely filled. A box that is "1" filled is
completely empty, and a box that is half filled is "0.5".
