The problem appears to be the scanned image. Once converted by
Ghostscript, it looks like the image I have attached.
This will not process well due to the incomplete lines in the barcode
and in the corner edges, and also in the boxes.
Although you may be setting your scanner to Monochrome and 300DPI, it
looks like the created PDF file is greyscale (256 colours) - I did a
colourcube analysis in GIMP to ascertain this. What software are you
using to scan these images?
This means that when Ghostscript converts it to a 1bit image, it cuts
out the parts of the image below a threshold and there will be gaps in
