Yeah, that's annoying :) I'll go ahead and underline something like
"y" or "p" in Search-as-you-type.

Hmm, a colon seems odd since the control is sort of in the middle of a
visual "sentence" -- I guess what I mean is that I think the colon
belongs after the dropdown menu, not before it.

Interesting. I don't see this effect here. What them are you using?

Hmm -- didn't know you could even put access keys there, but I'll look
into it. Of course this will increase competition for access keys
(though conflicts aren't the end of the world except in the case of
menus, where they become quite a PITA).

Well, typing C-f (or, if you have emacs-style keybindings set, C-s)
when in the index box gets you to the search box, but that's not
I'm thinking maybe I should rearrange the top to look something like this
Search _for [ ] _in [title ] Search as you t_ype =20
Adva_nced Search [Limit Search]
What do you think?

Not sure what you mean here. Do you mean the lefthand column of the
list (this is where images go when there are images, btw)? Putting
text in that label would be misleading, since that's where labels for
individual columns go, not for whole lists.
