This is probably a topic for the SMW user list.
Answer: in SMW 1.3, it could sometimes happen that data was associated with
the wrong page when being stored (this was happening on some wikis with
specific extensions or jobs running in the back). To prevent this from
happening again, upgrade to the new SMW 1.4.0.
To fix your existing problem in SMW 1.4.0, go to the page Special:SMWAdmin. It
contains a section "Data repair and upgrade". This section contains a button.
Press it. The repair proceeds slowly (to minimize load) but can be speeded up
as described on Special:SMWAdmin. There you can also monitor the progress of
the repair.
Note that this step is required anyway when upgrading to SMW 1.4.0.
If this still fails, send a bug report. Some more information on repairing SMW
data is given in the SMW administrator's handbook [1] -- the delete-and-
rebuild method included there should really cover all conceivable disasters
that could have affected the integrity of your SMW data (well, to the extent
that the disaster in question has left you with a running MediaWiki, which
certainly is the case for all disasters caused by any version of SMW ;-).
