I have an internal doc to produce that requires that sort of numbering.
I don't think it's worth any major amount of hacking, so I'll just hard
code the beast. Some kind of generalized substitution syntax for a
named enum-index would be cool, but probably not a prioirty for ReST
I seem to remember using some such things in TeX/Latex, but we *don't*
want ReST looking like TeX ;-) !
