maybe I misunderstand you, but "both inputs per channel" sounds
wrong. Channel 1 should have one of the inputs and channel 2 the other
one. It could explain a part of the problem.

This is strange. In case of a polarity problem I'd expect it to vanish
somewhere and reappear when your panning slider is at maximum.
Check your input routing first:
channel1 : X alsa_pcm_1
channel2 : alsa_pcm_1
If you have a mono microphone, better use a mono wav track, too.
