The form looks fine, except that you're missing the /cgi-bin/ part of the path for FormMail.pl, i.e. you need to change action="http://www.congodesign.com/FormMail.pl" to action="http://www.congodesign.com/cgi-bin/FormMail.pl" (assuming CGI scripts go in cgi-bin on you server) -- Nick
