Seems like your apache is misconfigured http://httpd.apache.org/docs/

You shouldn't have to do this.

Again, apache seems to be misconfigured, or apache/mod_perl were not built 
correctly. Did you build them *exactly* like the install documentation says to?

Open up index.shtml. Look at the files that it tries to include.
Are all of those files in the directory? It does not sound like it - and it 
doesn't sound like you have slashd running, which would make those header 
and footer files that index.shtml is trying to include.

Sounds like the slash.conf is not being loaded properly;  your
<VirtualHost> directive should be setting your DocumentRoot,
DirectoryIndex, and "AddHandler .inc" but apparently is not.

Run "apachectl configtest" and check there are no errors.  Look at
your httpd.conf to make sure the "Include /usr/local/slash/httpd/
slash.conf" is in there.  Look at slash.conf to make sure the
"Include /usr/local/slash/site/yoursitename/yoursitename.conf"
is in there.
