I am reviewing our files to help us clarify the meaning of macros in preparation for adding 64bit options.  Each file will have its own issue.  Macros that have MINGW32 in them are being scrutinized to determine if the 32 is meaningful to the definition of the macro such that it reflects 32bitness.  Please review and comment.

Summary: include/stdint.h needs __int64 when _WIN64 is defined

Initial Comment:
The inclusion of stdint.h causes an undefined type error if _WIN64 is defined.  The simplest solution is to include _mingw.h within this header so that __int64 is defined before it is used.
