miktex "observes" its own packages and "repairs" them when files in
this packages have been changed - this doesn't neeed to be fatal
changes. I think change dates or some file attributes can trigger
the "repair".

If you have a multiuser installation run the update manager in both
modes (admin and user mode).

If you have a network installation check if you have installed
things in a roaming profile.

Sure. I have already done it with a pen-drive. Start with a computer 
which *has* an internet connection. Go to:

http://www.miktex.org/2.9/setup

and click the "Download" button after "MiKTeX 2.9 Net Installer". Put 
the executable file that you get in the pen-drive. Then run that program 
and ask for a complete installation. It will download all packages. Tell 
the computer to put them in a folder at the pen-drive. Then put the 
pen-drive in the PC without internet connection in which you want to 
install MIKTeX, run the installer and tell it to get the packages from 
the folder in which you have put them.

No. But some people can ran into problems regarding the format

Not sure what the problem is, but this is what I do in such cases.

1) Ensure that the file that is missing is actually on the system, in the TeX system tree. 
2) Ensure that I have refreshed the FNDB
   Start->programs->MikTeX 2.9->Settings->Refresh FNDB
3) If file is not on system, try to google it, and you might find it online. Put it in a correct place, go to step 2 and try to rebuild format files.

dehypht-x-2009-06-19.tex no longer exists. The new version of the
"german-latest" pattern family has the date 2012-05-30 in its name.
So languages which uses the old version should be deactivated or
removed or changed.

On my PC the update removed the language. But I have a single user
installation. In multiuser system local settings can overwrite
global ones and if the local ones haven't been updated too old
settings can remain. 

Your log-file use a local configuration file:

> C:\Users\denise\AppData\Roaming\MiKTeX\2.9\tex\generic\config\language.def

At first run, if you haven't done it yet, an update in user mode.
Then check the language tab in miktex setting (in user + admin mode) 
and remove or deactivate languages with 2009-06-19 (there can be
more than one. Look for german, ngerman, swissgerman).

I opened the settings in user mode and removed the three languages as you suggested. As soon as I clicked the "Apply" button, a format update was automatically triggered which has resolved the issues.

I did install and now run miktex without any problems on a
restricted user account in win7 (in single user mode) and can also
make updates without any problems.
