Don't expose the "name" field to users

It's confusing, and most users don't care anyway. Because it affects the url, we should find some way to let advanced users edit it, but it doesn't need to be there for most users.

It needs to be removed from several places: Album creation, image editing, Photo Info block. Maybe others too that I'm not aware of.

Also, don't call it "name". Call it what it is, "URL", or "File name".
